Terms and Conditions

Treatment

​

  • You will only be assessed and treated by me, Celia Paterson. I am a qualified Physiotherapist who is registered with the Health Professional Council (HCPC) and the Chartered Society of Physiotherapist (CSP). My practice will be in line with their codes of practice.

  • Your consultation will be within the privacy of a treatment room. After examination I will explain the outcome of the assessment and the proposed treatment plan, including any risks. You may decline any of the treatments proposed without prejudice.

  • Payment will be due and taken on the day of treatment.

  • You must give me a minimum of 24 hours notice of cancellation of appointments. This can be done either by phone or email. If not, you will be liable to pay 50% of your appointment fee. This is to cover the cost of the room hire and potential lost earnings.

  • I am a private physiotherapist and do not have a contract with the NHS. I do not deal directly with insurance companies. However, I am happy to provide a receipt which you may be able to use to claim for any reimbursement.

  • Children under the age of 18 will be required to have a guardian present at the appointment. When there is an occasion this is not possible, there will need to be written consent prior to the appointment from the legal guardian. This will ideally be in the form of an email or a written note.

 

Personal Information

  • In order to perform my obligations to you as a professional physiotherapist I need to hold certain personal details about you, including your name, age, gender, medical history and details of any past, present and future treatment or advice provided. This will be sole purposes of maintaining up to date medical records.

  • If I need to contact another health professional or organisation on your behalf containing personal information e.g. GP letter or referral, I will gain specific consent from you first.

  • I will only use your email and/or telephone number to contact you in relation to an appointment or query.

  • I use a specialised clinical system called Nookal. They use encryption algorithms compliant with standard AES-256 during transmission and storage of data. Information is stored in servers in Ireland. For more information please go to www.nookal.com/uk/security

Insurance Patients

 

  • Patients need to contact the insurance company FIRST to ensure pre-authorization for physiotherapy and a set number of sessions. 

  • Patient must provide their insurance details, policy number and pre-authorisation code prior to physiotherapy treatment. If this does not occur or there is no authorization for any of the  treatment sessions, the patient will be liable to self fund the Southridge Physio's full price for that treatment 

  • Southridge Physio is unable to obtain or confirm policy or authorisation details on your behalf. It is the patient’s responsibility to contact their Insurance Company directly for conditions of their policy, limitations including excess amount prior to booking. 

  • The invoiced policy excess amount is the responsibility of the insured patient to self fund promptly and directly to Southridge Physio. Any issues with the amount should be raised directly with the insurance company after full payment to Southridge Physio. A receipt can be provided to claim back.  Once policy excess amount has been reached the insurance company will start to pay. 

  • If your Health Insurance Physiotherapy limit has been met and no further treatment is authorised, you may continue to receive Physiotherapy on a ‘self-pay’ basis, the Physiotherapy fees will revert to the normal treatment price as per our Treatment Tariff.

  • Please note Insurance appointments are limited to only 30mins.
